This photo, taken from the south bank of the Kokosing River, shows the collapsed bridge on Route 62 in Millwood.

US 62 originally approached the bridge from the southeast (bottom right of the photo), curving to make a 90-degree crossing over the Kokosing River. Millwood Road intersects from the left.

Horton either fell asleep or was distracted, for he failed to negotiate the curve into the bridge, striking the original span approximately where the modern hazard sign marks the current structure. Even if he had negotiated this curve, it can be seen that the curve on the far side of the river was even more treacherous. Had the trucker maintained any speed at all, the second curve would have been impossible.

Due to the fiery accident, US 62 was routed slightly east of Millwood. Its higher and longer bridge can be seen in the background of this shot, slightly downriver from the older span.

The earliest known photo from the wreck shows Horton’s semi-truck cab still sitting on the bridge, where it was consumed in intense flames.
