MOUNT VERNON — Knox County Community Health Center has achieved National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) Recognition, a distinction that reflects the health center’s commitment to providing coordinated, accessible, and patient-centered care.
The recognition highlights key areas of care, including team-based care, patient-centered access and continuity, care management and support, care coordination and transitions, knowing and managing patients, and performance measurement and quality improvement.
The PCMH model places patients at the center of their care by strengthening relationships between patients and their clinical care teams. Research shows that PCMHs can improve quality, patient experience, and staff satisfaction, while helping reduce health care costs. Practices that earn recognition demonstrate a commitment to quality improvement and a patient-centered approach to care.
“Receiving this recognition reflects the dedication of our entire team to providing compassionate, coordinated care for every patient we serve,” Project Director Anthony Bowersock said. “We’re proud to offer a care model that focuses on building lasting relationships while helping our patients achieve better health outcomes.”
NCQA is a private, nonprofit organization dedicated to improving health care quality. It accredits and certifies a wide range of health care organizations.
NCQA also recognizes clinicians and practices in key areas of performance. NCQA’s Healthcare Effectiveness Data and Information Set (HEDIS®) is the most widely used performance measurement tool in health care.
The agency offers information to help consumers, employers and others make informed health care choices.
